DS-82 can be mailed without appearing in person. DS-11 is for everyone else — first timers, minors, and those whose prior passport doesn't meet those criteria. DS-11 always requires an in-person appearance.

What are the rules for minors under 16?

Children under 16 must apply in person with both parents or legal guardians present. If one parent cannot attend, the absent parent submits Form DS-3053 (Statement of Consent), notarized.
