Form 4 is the Application for Licence to Drive a Motor Vehicle — the form you file to get a permanent driving licence in India under Section 9 of the Motor Vehicles Act, 1988 and Rule 14 of the Central Motor Vehicles Rules, 1989. Form 2 is the application for a learner licence — the first step, which lets you practise driving under supervision after passing a computer-based test on traffic rules. Form 4 is the application for the permanent driving licence — the second step, filed at least 30 days after your learner licence is issued and within 180 days of its issue, and it leads to the practical driving skill test at the RTO. If you have never held any licence, start with Form 2; if you already hold a valid learner licence, Form 4 is the form you need.

It depends on your age and vehicle class. For a non-transport (private) licence applied for under age 40, a self-declaration of physical fitness on Form 1 is normally enough — no doctor visit needed. A medical certificate on Form 1A, signed by a registered medical practitioner, is required if you are applying for a transport vehicle licence at any age, or if you are aged 40 or above. Some states also ask for Form 1A for specific endorsements, so check your RTO's checklist before you go.

You must wait at least 30 days from the date your learner licence was issued before filing Form 4, and you must apply within 180 days — the learner licence is valid for six months. If the 180-day window lapses, the learner licence expires and you have to start over with a fresh Form 2 application, another test of traffic rules, and fresh fees. Most applicants book the driving test slot for shortly after the 30-day mark so there is comfortable buffer inside the window.

The standard checklist: your valid learner licence (original), proof of age (birth certificate, Class 10 marksheet, passport or PAN card), proof of address (Aadhaar, passport, voter ID, utility bill or rent agreement), two to three recent passport-size photographs, the Form 1 physical-fitness self-declaration, and Form 1A medical certificate where applicable (age 40+ or transport vehicles). Carry originals for verification plus one photocopy set. Some RTOs additionally ask for proof of the driving-school certificate (Form 5) for transport-class applications.

States add their own charges — smart-card fees of roughly Rs 200, service or facilitation charges, and separate fees per additional class of vehicle — so the all-in cost typically lands between Rs 700 and Rs 1,500 depending on the state and how many vehicle classes you apply for. The Motor Vehicle Inspector checks documents, then tests basic control: for two-wheelers typically a figure-of-eight and slow ride; for cars an H-track or 8-track with reversing, plus signals and observation.
