Form 4 is the application for the permanent driving licence — the second step, filed at least 30 days after your learner licence is issued and within 180 days of its issue, and it leads to the practical driving skill test at the RTO. If you have never held any licence, start with Form 2; if you already hold a valid learner licence, Form 4 is the form you need.

Do I need a medical certificate (Form 1A) with Form 4?

It depends on your age and vehicle class. For a non-transport (private) licence applied for under age 40, a self-declaration of physical fitness on Form 1 is normally enough — no doctor visit needed. A medical certificate on Form 1A, signed by a registered medical practitioner, is required if you are applying for a transport vehicle licence at any age, or if you are aged 40 or above. Some states also ask for Form 1A for specific endorsements, so check your RTO's checklist before you go.

When can I apply for a permanent licence after getting my learner licence?

You must wait at least 30 days from the date your learner licence was issued before filing Form 4, and you must apply within 180 days — the learner licence is valid for six months. If the 180-day window lapses, the learner licence expires and you have to start over with a fresh Form 2 application, another test of traffic rules, and fresh fees. Most applicants book the driving test slot for shortly after the 30-day mark so there is comfortable buffer inside the window.

What documents do I need to submit with Form 4?

The standard checklist: your valid learner licence (original), proof of age (birth certificate, Class 10 marksheet, passport or PAN card), proof of address (Aadhaar, passport, voter ID, utility bill or rent agreement), two to three recent passport-size photographs, the Form 1 physical-fitness self-declaration, and Form 1A medical certificate where applicable (age 40+ or transport vehicles).
